​We can assist in securing your Passport Application schedule:

CONDITIONS
1. We can secure passport schedules via online but cannot guarantee when slots are available. 
2. Pre-payment is required since we don't know when schedules come up. 
3. We cannot cater to specific schedule request as schedules are given based on earliest availability.
4. No guarantee of same time and date schedule for groups/family. 
5. In case of cancellation of booking/change of mind (before appointment set), Southwind will charge P500.00 service fee. 
6. Non-refundable if cancelled after DFA schedule has been finalized.  Rebooking is not allowed once finalized. If cancelled, you may schedule new appointment and pay passport fees again. 

If you Agree to all conditions stated:
1.  Fill out the Application Form (link below) and Submit.
2. Wait for acknowledgement of email for payment instructions. 
3. Once an appointment is secured, we will revert via email or text/call mobile number indicated.

5. After securing a confirmed slot, we will send by email the Barcoded Form from DFA and reminders on what to bring and print.
